Please respect your time and other patients by arriving for your appointment on time.

Empirical Point requires a cancellation of a minimum of 24 hours prior to your appointment to avoid being charged the full cost of the appointment. Your cooperation and consideration are greatly appreciated.

New Patient Information Form

Please print out a copy of the New Patient Information Form and fill it out as completely as you can prior to your first appointment. Completing the form may take 30-45 minutes and is best done in advance.

If you have other important medical information you think is relevant, please bring it with you to your appointment.

If a physician or other health professional has referred you, please bring contact information for that provider, so that, with your permission, and if appropriate, we can send progress reports that will help enhance your overall treatment regimen and care.
